    ABSTRACT: In developed countries, diabetes is one of the ten most common causes of death. Post mortem diagnosis of glucose metabolism disorders can be difficult and vague because of the lack of characteristic morphological findings. Reviews of the literature are presented concerning biochemical problems in cases of unclear hyper- or hypoglycemia. After repetition of causes, frequency, and mortality of diabetic metabolism disorders, we give hints for the detection of diabetic ketoacidosis, hyperosmolar coma, insulinoma, and insulin- or oral diabetic-induced hypoglycemia. The first part discusses the analytes glucose and lactate, glycated proteins and oral antidiabetics, with special regard to their matrices post mortem, to reference concentrations, stability data and to analytic procedures that should be used in clinical or toxicological laboratories to detect diabetic metabolism disorders after death.

    ABSTRACT: Die Möglichkeiten des Missbrauchs von Dopingsubstanzen im Sport nehmen mit neuen Medikamenten und Methoden stetig zu. Die Nebenwirkungen von Anabolika und Stimulanzien sind detailliert untersucht, und auch von den neueren Peptidhormonen sind unerwünschte Effekte von Patienten bekannt. Einige Fallstudien beschreiben die tödlichen Folgen durch Dopingsubstanzen und die Obduktionsbefunde, die sich auf die Einnahme bestimmter Substanzen oder Substanzklassen zurückführen ließen. Aufgrund der Kombination unterschiedlicher Substanzen und der Applikation supraphysiologischer Mengen sind Wirkungen und Nebenwirkungen jedoch häufig nicht absehbar, und im Todesfall ist es schwer, anhand der bei der Obduktion vorliegenden organischen Schäden Rückschlüsse auf die eingenommenen Substanzen zu ziehen. The possibilities of misusing doping substances in sports, continuously increases with the development of new medicinal substances. Undesirable effects of anabolic steroids and stimulants have been well investigated and undesirable side effects related to peptide hormones are known from patients. Case reports about doping-induced deaths have described the findings of autopsies, which indicated the involvement of certain classes of substances and drugs. Nevertheless, the increasing abuse of combinations of different compounds complicates the appraisal of side effects. Therefore, evaluation of autopsy results after deaths related to doping has become a complex task and alterations in tissues and organs are often difficult to attribute to a certain class of administered substances.
