ABSTRACT This review summarizes the anatomic and biomechanical features of the thoracic spine, which are different from those of the more mobile segments of the spine, and emphasizes their role in trauma. The distinguishing characteristics of the thoracic spine are the presence of the ribs and their articulations. The rib cage restricts motion and adds stiffness to the spine. During trauma, it provides the thoracic spine with additional strength and energy-absorbing capacity. Above the T10 level, most injuries produce a basic pattern consisting of an anterior fracture-dislocation involving two contiguous vertebrae, often with associated neurologic impairment. The definition of spinal instability remains controversial. CT is the imaging technique of choice for evaluation of spine fractures; however, MR imaging is superior in the evaluation of spinal cord injury and posttraumatic disk herniation. MR imaging also provides prognostic information not obtainable with other imaging methods.

    ABSTRACT: Traumatic injury of the thoracic aorta is a major clinical concern in patients who sustain deceleration or crush injuries. Several mechanical factors may explain the typical locations of thoracic aortic rupture (aortic isthmus, ascending aorta). Understanding these factors and the pathophysiology involved helps the radiologist to recognize aortic trauma at various imaging examinations. Chest radiography is the initial screening examination, and radiographs are evaluated specifically for signs of mediastinal hematoma, an indication of significant thoracic trauma. The most important of these signs include loss of aortic contour, tracheal deviation, ratio of mediastinal width to chest width, deviation of a nasogastric tube to the right of the T-4 spinous process, and depression of the left main-stem bronchus (> 40 degrees below the horizontal). Computed tomography (CT) is used increasingly when results of chest radiography are equivocal. CT can clearly demonstrate mediastinal hematoma, but this finding is also mimicked by several entities, including atelectatic lung, thymus, and pericardial recesses. Aortography is the standard for diagnosis. Traumatic aortic injury is treated urgently with surgical repair. The rare patient who survives aortic injury without surgery may develop a chronic pseudoaneurysm.

We will review the pertinent anatomy of the costovertebral articulation, summarize the current literature, report our experience and the imaging appearance of costovertebral joint fracture dislocations, and discuss important concepts of the costovertebral joint, as it relates to thoracic spine fractures.

    ABSTRACT: Objective: the traumatic lesions of the thoracic spine (T1-T-10) are influenced by the segment's peculiar characteristics. The goal of this study is to evaluate surgically treated patients, for unstable fractures, via posterior approach and fixation. Methods: retrospective study of 11 patients, age varying from 22 to 52 years, submitted to posterior fixation and arthrodesis. The patients were studied pre-surgery, immediate post-surgery and after two years of follow-up, by parameters such as clinical (pain and neurological status), radiological (cifosis of the fracture segmental, arthrodesis consolidation, disorders of the surgical instrumental) and functional (Denis' work Grade and SF-36). Results: the patients were studied in a period of time varying from 24 to 29 months (average 27.7). Two (18.18%) patients didn't have pain, three (27.27%) patients had little pain, and three patients had a strong pain.
