A Randomized Trial of Improved Weight Loss With a Prepared Meal Plan in Overweight and Obese Patients

To assess the long-term effects of a prepackaged, nutritionally complete, prepared meal plan compared with a usual-care diet (UCD) on weight loss and cardiovascular risk factors in overweight and obese persons.
In this randomized multicenter study, 302 persons with hypertension and dyslipidemia (n = 183) or with type 2 diabetes mellitus (n = 119) were randomized to the nutrient-fortified prepared meal plan (approximately 22% energy from fat, 58% from carbohydrate, and 20% from protein) or to a macronutrient-equivalent UCD.
The primary outcome measure was weight change. Secondary measures were changes in blood pressure or plasma lipid, lipoprotein, glucose, or glycosylated hemoglobin levels; quality of life; nutrient intake; and dietary compliance.
After 1 year, weight change in the hypertension/dyslipidemia group was -5.8+/-6.8 kg with the prepared meal plan vs -1.7+/-6.5 kg with the UCD plan (P<.001); for the type 2 diabetes mellitus group, the change was -3.0+/-5.4 kg with the prepared meal plan vs -1.0+/-3.8 kg with the UCD plan (P<.001) (data given as mean +/- SD). In both groups, both interventions improved blood pressure, total and low-density lipoprotein cholesterol levels, glycosylated hemoglobin level, and quality of life (P<.02); in the diabetic group, the glucose level was reduced (P<.001). Compared with those in the UCD group, participants with hypertension/dyslipidemia in the prepared meal plan group showed greater improvements in total (P<.01) and high-density lipoprotein (P<.03) cholesterol levels, systolic blood pressure (P<.03), and glucose level (P<.03); in participants with type 2 diabetes mellitus, there were greater improvements in glucose (P =.046) and glycosylated hemoglobin (P<.02) levels. The prepared meal plan group also showed greater improvements in quality of life (P<.05) and compliance (P<.001) than the UCD group.
Long-term dietary interventions induced significant weight loss and improved cardiovascular risk in high-risk patients. The prepared meal plan simultaneously provided the simplicity and nutrient composition necessary to maintain long-term compliance and to reduce cardiovascular risk.

Diabetes occurs when the pancreatic beta cells are either unable to produce enough insulin, or the body cells were unable to use available insulin effectively. Insulin, a storage hormone produced in the pancreatic beta cells, helps to absorb glucose for the production of energy during cellular respiration. Thus, failure of absorbing glucose by body cells results in the accumulation of glucose in the blood, which is termed as hyperglycemia. Excess blood glucose causes serious damage to the major organ systems, leading to the heart disease, kidney failure (nephropathy), blindness (retinopathy), loss of sensation in the feet and hands and even an early death. Thus, diabetes is a chronic condition where the body's ability to convert food into energy is impaired. Prevalence of diabetes along with dyslipidaemia is increasing at an explosive rate throughout the globe. Though the exact cause is unknown, many factors are believed to play roles in this pathogenesis. Genetic predispositions, faulty dietary patterns, and sedentary life-styles are the key factors for the increased prevalence of type 2 diabetes. This progressive disease develops through many years, and pre-diabetic condition is known to be reversible. Though antidiabetic drugs are the first line of defence to control blood glucose level, conventional therapy is unable to cure it, nor can prevent the long term damage of the vital systems, but needs to be used lifelong. On the other hand, the benefit of nutritional medicine and dietary supplementation in diabetes is less explored scientifically. Therefore, the safer alternatives to prevent, or minimize the long term damage and control of diabetes need a combination of healthy eating, regular physical activity, monitoring of blood glucose level, dietary control and nutritional medicine. This review will cover not only the different aspects of diabetes, but also the role of complementary personalized therapy using nutrition and nutritional medicine in its management, highlighting a proper blend of conventional therapy with nutraceuticals, for effective diabetes management.

    ABSTRACT: Obesity has become a global epidemic over the past few decades because of unhealthy dietary habits and reduced physical activity. Hypertension and diabetes are quite common among obese individuals and there is a linear relationship between the degree of obesity and these diseases. Lifestyle interventions like dietary modifications and regular exercise are still important and safe first-line measures for treatment. Recently, bariatric surgery has emerged as an important and very effective treatment option for obese individuals especially in those with comorbidities like hypertension and diabetes. Though there are few effective drugs for the management of obesity, their efficacy is only modest, and they should always be combined with lifestyle interventions for optimal benefit. In this paper we aim to outline the non-pharmacological measures for the management of hypertension and diabetes in obesity.

    ABSTRACT: The pandemic of overweight and obesity continues to rise in an alarming rate in western countries and around the globe representing a major public health challenge in desperate need for new strategies tackling obesity. In the United States nearly two thirds of the population is overweight or obese. Worldwide the number of persons who are overweight or obese exceeded 1.6 billion. These rising figures have been clearly associated with increased morbidity and mortality. For example, in the Framingham study, the risk of death increases with each additional pound of weight gain even in the relatively younger population between 30 and 42 years of age. Overweight and obesity are also associated with increased co-morbid conditions such as diabetes, hypertension and cardiovascular disease as well as certain types of cancer. In this review we discuss the epidemic of obesity, highlighting the pathophysiologic mechanisms of weight gain. We also provide an overview of the assessment of overweight and obese individuals discussing possible secondary causes of obesity. In a detailed section we discuss the currently approved therapeutic interventions for obesity highlighting their mechanisms of action and evidence of their efficacy and safety as provided in clinical trials. Finally, we discuss novel therapeutic interventions that are in various stages of development with a special section on the weight loss effects of anti-diabetic medications. These agents are particularly attractive options for our growing population of obese diabetic individuals.
