Real-time, evidence-based medicine instruction: a randomized controlled trial in a neonatal intensive care unit.

ABSTRACT The study assesses potential for improving residents' evidence-based medicine searching skills in MEDLINE through real-time librarian instruction.
Ten residents on a rotation in a neonatal intensive care unit participated.
Residents were randomized into an instruction and a non-instruction group. Residents generated questions from rounds and searched MEDLINE for answers. Data were collected through observation, search strategy analysis, and surveys. Librarians observed searches and collected data on questions, searching skills, search problems, and the test group's instruction topics. Participants performed standardized searches before, after, and six-months after intervention and were scored using a search strategy analysis tool (1 representing highest score and 5 representing lowest score). Residents completed pre- and post-intervention surveys to measure opinions about MEDLINE and search satisfaction.
Post-intervention, the test group formulated better questions, used limits more effectively, and reported greater confidence in using MEDLINE. The control group expressed less satisfaction with retrieval and demonstrated more errors when limiting. The test and control groups had the following average search scores respectively: 3.0 and 3.5 (pre-intervention), 3.3 and 3.4 (post-intervention), and 2.0 and 3.8 (six-month post-intervention).
Data suggest that measurable learning outcomes were achieved. Residents receiving instruction improved and retained searching skills six-months after intervention.

    ABSTRACT: Medical residents are a distinct group of health sciences library users that need detailed instruction in literature searching. Core residency competencies from the Accreditation Council for Graduate Medical Education require that all residents be competent in finding, critically appraising, and incorporating the latest clinical evidence into their daily practice with patients. With the collaboration of the library and a clinical department of pediatrics, a program was developed that teaches residents the basic elements of EBM searching and then reinforces this learning over the course of three years.

    ABSTRACT: Internationally, the hospital library serves the Hospital as the primary department responsible for the provision of the Knowledge-based Information and the development of systems and services in order to meet the Knowledge-based Information needs of the Organization. Standard 5 of the Medical Library Association (Standards for hospital libraries 2002 with 2004 revision) demonstrates that there is evidence for effective connections between Knowledge-based Information (KBI) and patient care and education for medical and hospital staff. Concerning the education of medical and hospital staff, the role of the library by the medical librarian must include: the participation of the librarian in the team directing the educational functions in the hospital, the regular provision of material in support of planning and preparing of educational activities, the education of medical and hospital staff on information management and use of information technology, the identification of print and electronic resources for individualised learning, joint library and continuing education, the provision of specific literature in support of educational activities, the provision of access to quality resources of continuing education such as Web-based resources. The aim of this study is to investigate and evaluate the provided user education and training for hospital and medical staff in the Greek hospital libraries and the participation of librarian in the educational activities of institution. Sample of this survey was Greek hospital libraries. Questionnaires were sent out to 44 hospital libraries of Greece and Cyprus. 27 questionnaires were returned, a response rate of 61%. The survey was carried out in April-May 2008 The findings indicate that Greek hospital libraries provide user education, however, it has informal character. The librarian does not participate in the team directing educational functions in the hospital and continuing education resources are not provided. Consequently, either due to unsatisfactory financial support and to well- built stereotypes or due to personnel deficiency and to institutional shortage, MLA standards are being met by inadequate way.

    ABSTRACT: Information and communication technologies (ICT) are defined as digital and analogue technologies that facilitate the capturing, processing, storage and exchange of information via electronic communication. ICTs have the potential to improve information management, access to health services, quality of care, continuity of services, and cost containment. Knowledge is lacking on conditions for successful ICT integration into practice. To carry out a systematic review of the effectiveness of interventions to promote the adoption of ICT by healthcare professionals. Specific strategies, defined with the help of an information specialist, were used to search the Cochrane Effective Practice and Organisation of Care Group (EPOC) register and additional relevant databases. We considered studies published from January 1990 until October 2007. Randomised controlled trials (RCTs), controlled clinical trials (CCTs), controlled before/after studies (CBAs), and interrupted time series (ITS) that reported objectively measured outcomes concerning the effect of interventions to promote adoption of ICT in healthcare professionals' practices. Two reviewers independently assessed each potentially relevant study for inclusion. We resolved discrepancies by discussion or a third reviewer. Two teams of two reviewers independently extracted data and assessed the quality of included studies. A meta-analysis of study outcomes was not possible, given the small number of included studies and the heterogeneity of intervention and outcomes measures. We conducted qualitative analyses, and have presented the results in a narrative format. Ten studies met the inclusion criteria. Nine of them were RCTs. All studies involved physicians as participants (including postgraduate trainees), and one study also included other participants. Only two studies measured patient outcomes. Searching skills and/or frequency of use of electronic databases, mainly MEDLINE, were targeted in eight studies. Use of Internet for audit and feedback, and email for provider-patient communication, were targeted in two studies. Four studies showed small to moderate positive effects of the intervention on ICT adoption. Four studies were unable to demonstrate significant positive effects, and the two others showed mixed effects. No studies looked at the long-term effect or sustainability of the intervention. There is very limited evidence on effective interventions promoting the adoption of ICTs by healthcare professionals. Small effects have been reported for interventions targeting the use of electronic databases and digital libraries. The effectiveness of interventions to promote ICT adoption in healthcare settings remains uncertain, and more well designed trials are needed.
