Article: Hepatic fat quantification: a prospective comparison of magnetic resonance spectroscopy and analysis methods for chemical-shift gradient echo magnetic resonance imaging with histologic assessment as the reference standard.

ABSTRACT: The aims of this study were to assess the confounding effects of hepatic iron deposition, inflammation, and fibrosis on hepatic steatosis (HS) evaluation by magnetic resonance imaging (MRI) and magnetic resonance spectroscopy (MRS) and to assess the accuracies of MRI and MRS for HS evaluation, using histology as the reference standard. In this institutional review board-approved prospective study, 56 patients gave informed consents and underwent chemical-shift MRI and MRS of the liver on a 1.5-T magnetic resonance scanner. To estimate MRI fat fraction (FF), 4 analysis methods were used (dual-echo, triple-echo, multiecho, and multi-interference), and MRS FF was calculated with T2 correction. Degrees of HS, iron deposition, inflammation, and fibrosis were analyzed in liver resection (n = 37) and biopsy (n = 19) specimens. The confounding effects of histology on fat quantification were assessed by multiple linear regression analysis. Using the histologic degree of HS as the reference standard, the accuracies of each method in estimating HS and diagnosing an HS of 5% or greater were determined by linear regression and receiver operating characteristic analyses. Iron deposition significantly confounded estimations of FF by the dual-echo (P < 0.001) and triple-echo (P = 0.033) methods, whereas no histologic feature confounded the multiecho and multi-interference methods or MRS. The MRS (r = 0.95) showed the strongest correlation with histologic degree of HS, followed by the multiecho (r = 0.92), multi-interference (r = 0.91), triple-echo (r = 0.90), and dual-echo (r = 0.85) methods. For diagnosing HS, the areas under the curve tended to be higher for MRS (0.96) and the multiecho (0.95), multi-interference (0.95), and triple-echo (0.95) methods than for the dual-echo method (0.88) (P ≥ 0.13). The multiecho and multi-interference MRI methods and MRS can accurately quantify hepatic fat, with coexisting histologic abnormalities having no confounding effects.Investigative radiology 04/2012; 47(6):368-75. · 4.85 Impact Factor -

Article: Cocaine use in the past year is associated with altitude of residence.

ABSTRACT: : Recently, increased rates of suicide in US counties at higher altitudes have been noted. Because of the documented association between cocaine use and suicide, we hypothesized that there would be a correlation between incidence of cocaine use and altitude of residence. : Cocaine use data were obtained from the Substate Substance Abuse Estimates from the 1999-2001 National Surveys on Drug Use and Health. Data related to the percentages of people 12 years or older who used cocaine in the past year. Average elevation for US counties was calculated using the Shuttle Radar Topography Mission elevation data set, and subject region elevation was calculated by averaging the weighted elevations of each region's relevant counties. The correlation between elevation of a substate region and incidence of cocaine use in that region was calculated using Pearson correlation coefficients. : A significant correlation exists between mean altitude of a substate region and incidence of cocaine use in that region (r = 0.34; P < 0.0001). Regression analysis controlling for age, sex, race, education level, income, unemployment, and population density was performed. Altitude remained a significant factor (P = 0.007), whereas male sex (P = 0.008) and possessing less than a college education (P < 0.0001) were also significant predictors of self-reported cocaine use in the past year. It is important to note that cocaine use was assessed in isolation of other drugs of abuse, an additional confounding variable. : This study demonstrates a significant correlation between altitude of substate region of residence and incidence of cocaine use. It is possible that stress response due to hypoxia is responsible; however, this requires further investigation. However, because other substance use was not assessed, specificity of this association is unknown. Article: Volumetric, planar, and linear analyses of pharyngeal airway change on computed tomography and cephalometry after mandibular setback surgery.

ABSTRACT: The purpose of this study was to use lateral cephalometry and computed tomography (CT) to examine the volumetric, planar, and linear changes in the pharyngeal airway after mandibular setback surgery. The pharyngeal airways of 12 subjects who underwent mandibular setback surgery at Seoul National University Dental Hospital were assessed linearly and volumetrically on lateral cephalometric radiographs and CT before surgery and 6 months after surgery. The pharynx, nasopharynx, and oropharynx were evaluated by volumetric analysis. Pharyngeal depth, airway space, pharyngeal soft-tissue thickness, and hyoid bone position were measured by linear analysis. The axial section area of the airway was measured by area analysis. From the linear analysis, a significant decrease in pharyngeal depth and a significant posterior movement of the hyoid bone (P <0.05) were noted. Volumetric analysis by CT showed that the oropharynx decreased after mandibular setback surgery. However, the volume and the axial section area of the airway in the CT images did not change significantly after mandibular setback surgery (P >0.05). Article: Improved correlation between CT emphysema quantification and pulmonary function test by density correction of volumetric CT data based on air and aortic density.

ABSTRACT: OBJECTIVES: To determine the improvement of emphysema quantification with density correction and to determine the optimal site to use for air density correction on volumetric computed tomography (CT). METHODS: Seventy-eight CT scans of COPD patients (GOLD II-IV, smoking history 39.2±25.3 pack-years) were obtained from several single-vendor 16-MDCT scanners. After density measurement of aorta, tracheal- and external air, volumetric CT density correction was conducted (two reference values: air, -1000HU/blood, +50HU). Using in-house software, emphysema index (EI) and mean lung density (MLD) were calculated. Differences in air densities, MLD and EI prior to and after density correction were evaluated (paired t-test). Correlation between those parameters and FEV(1) and FEV(1)/FVC were compared (age- and sex adjusted partial correlation analysis). RESULTS: Measured densities (HU) of tracheal- and external air differed significantly (-990±14, -1016±9, P<0.001). MLD and EI on original CT data, after density correction using tracheal- and external air also differed significantly (MLD: -874.9±27.6 vs. -882.3±24.9 vs. -860.5±26.6; EI: 16.8±13.4 vs. 21.1±14.5 vs. 9.7±10.5, respectively, P<0.001). The correlation coefficients between CT quantification indices and FEV(1), and FEV(1)/FVC increased after density correction. The tracheal air correction showed better results than the external air correction. CONCLUSION: Density correction of volumetric CT data can improve correlations of emphysema quantification and PFT.European journal of radiology 05/2012; · 2.65 Impact Factor -

Article: Panoramic endoluminal display with minimal image distortion using circumferential radial ray-casting for primary three-dimensional interpretation of CT colonography

ABSTRACT: The purpose of this study was to develop a panoramic endoluminal display technique, the band view, which causes minimal image distortion, and to determine its feasibility as a time-efficient primary three-dimensional review method of CT colonography (CTC). Image distortion was compared between the band view and four other three-dimensional review modes using ten 10-mm and ten 20-mm electronically generated polyps. Diagnostic performance and interpretation time were compared between the band view and the conventional endoluminal view by two independent readers in 52 patients who underwent CTC and colonoscopy on the same day. Mean image distortion index values, in which 1 indicates no distortion and the larger value represents greater distortion, were significantly smaller with the band view (1.03 and 1.01 for 10-mm and 20-mm polyps, respectively) than with the filet view (1.65 and 1.55) or the virtual colon dissection (3.27 and 3.85) (P ≤ 0.004). The sensitivity and specificity for detecting adenomatous polyps ≥6mm did not differ, but the mean interpretation time was significantly shorter with the band view than with the conventional endoluminal view by 1.8 and 4.5 minutes in readers 1 and 2, respectively (P < 0.0001).
