Dr. Bibhuti thanks for sharing the details of the case. As noted, even with a very conservative protocol, it is interesting to see that the problems occured post exercise rather than during. I had a patient a few months back (around the time I posted this question) who had a severe bradycardia following exercise. From a peak HR of 179 it dropped to 43 with no signs of heart block or ectopics.Her HR resolved to a baseline of around 60 within 5 min. - I guess we need more studies in this area. The risk of testing Eisenmenger's is indeed debatable. Dr.Guido, thank you for your comments. I am of the opinion that CPX (cardiopulmonary exercise testing) should form part of the clinical decision making as studies in PAH have shown that these CPX parameters do have prognostic implications. However, its use in children is still not known. Perhaps till we have solid evidence for the use of CPX in these populations, it will be the 6MWT that we would end up using safely. Like Dr. Bhibuti says, this is indeed worth investigating in future trials.Following

However, I would believe that irrespective of the situation, when it comes down to being an author on the paper, one should probably adhere to the ICMJE requirements. http://www.icmje.org/ethical_1author.html Most journals require you to highlight the contribution of each author on the paper during the submission process. This helps prevent "gift authorship" In my opinion, if your doctoral committee has been very closely linked to your entire project and dissertation with regard to planning and design, literature search, systematic reviews etc, then it would be right to include them. However, if they were only checking up on your work and not providing much input, I do not think authorship comes into the picture. One way to avoid this is by listing the order of authors before the paper is written and the order being agreed upon by all. This will solve some of the issues. Nevertheless, if you are the student, it is going to be difficult -but NOT impossible!Following

Ventricular arrhythmias are indeed a serious risk during CPX. Like you mention, I agree that the 6MWT probably stresses the body to a great extent in these already severely de-conditioned patients. The pathophysiology of Eisenmenger's is definitely to be considered during testing and we should work in this area to establish some facts. Dr. Das, if I may ask, what method and protocol of testing did you use for the patient you mentioned in your comment? Was a 6MWT done prior to the exercise test?Following

Article: Effect of exercise training on functional capacity & quality of life in head & neck cancer patients receiving chemoradiotherapy.

ABSTRACT: Background & objectives: Patients with head and neck cancer (HNC) undergoing chemoradiotherapy (CRT) suffer from fatigue causing a decrease in functional capacity and quality of life (QoL). Limited research in the field of exercise training among these patients demanded the need for this study to assess the effects of exercise training on functional capacity and quality of life. Methods: A randomized controlled trial was conducted on 48 patients with HNC undergoing CRT. The exercise group received an individually tailored, supervised, exercise programme for six weeks, while the control group did not receive any form of exercise. Functional capacity and QoL were assessed at baseline and at the end of the intervention using the six minute walk distance (6MWD) and medical outcomes survey short form 36 (SF 36). Results: The mean age of patients was 52 yr with 42 males. After six weeks, the 6MWD improved by 42 metres (P<0.05) in the exercise group while the control group showed a decrease by 96 metres (P<0.001). There was an improvement on the mental component score (MCS) of SF36 for the exercise group (4.8; P<0.05) and the physical component score (PCS) remained almost the same, while a decrease in PCS and MCS was seen in the control group (-5.9; P=0.064 and -17.3; P<0.05). When 6MWD and SF36 were compared between the groups, there was a statistically significant difference (P<0.001) seen after six weeks. Interpretation & conclusions: Our results showed that the functional capacity and QoL decreased among those not receiving a supervised exercise program, while exercise training improved functional capacity and QoL in HNC patients undergoing CRT.The Indian journal of medical research 03/2013; 137(3):515-20. · 1.84 Impact Factor -

ABSTRACT: Editorial Physiotherapy has come a long way since its inception during World War II. Various specialisations and specific treatment strate-gies have transformed the physiotherapist into a specialised clinician. With the growth of professional autonomy in physiotherapy practice, the extended scope of practice came into existence. As the science be-hind physiotherapy advances, it is expected that physiotherapists will continue to extend their scope of practice through role enhancement and substitution [1]. Extended scope of practice has been defined by the Chartered Society of Physiotherapists as "clinical physiotherapy specialists in any recognised speciality with an extended scope of practice" [2]. With growing emphasis in respiratory care to improve patient outcomes in the acute medical and surgical setting, extending physiotherapy services beyond regular working hours has the poten-tial to contribute towards better clinical outcomes. On-call physiotherapy or emergency physiotherapy started gain-ing popularity in the 1980s. However, it was not until 2000 that the term on-call physiotherapy was adopted. This has been defined as "the provision of respiratory/cardiorespiratory/cardiothoracic or combi-nations of respiratory and orthopedic physiotherapy, out of working hours" [3]. The earliest study identified was done by Holden and Daniele in 1987 where they found that five day and seven day physiotherapy ser-vices, for patients with acute orthopedic problems, could be provided with the existing staffing and without affecting the number of treat-ment sessions [4].Journal of Novel Physiotherapies. 01/2012;
