In formulating a questionnaire we have to identified what the construct of our subject or research title and goals. All of item in the questionnaire are belong to the construct of research concepts.
-
You have to consider all the clinical parameters of both diabetic and tuberculosis in your questionnaire because your research goal is on treatment of tuberculosis.
-
my exact title is 'prospective study on tuberculosis treatment outcome in diabetic patients registered under RNTCP in Bangalore.

It will be interesting and hope you can get reasonable nuber of cases for study. May be the drug interaction of anti-diabetic and antitubercular could be studied. One can find out the incidence of ADR among diabetics when treated for tuberculosis. May take a look.

Your questionnaire may include questions on:
*Demographics
*Presence or absence of diabetes: main predictor
*Clinical characteristics: weight and height of the patient (BMI), because well nourished patients may have a better outcome, type of diabetes, control of diabetes, duration of diabetes, co-morbid illnesses such as hypertension, type of drug used for treatment of diabetes, etc...
*Type of TB, Duration, Complications of TB, Type of drug used for treatment, co-morbid pneumonia or HIV etc..
*Quality of TB care provided by the facility
*Adherence of the patient to treatment -
Very intersting research, if the survey will be done to pacients it is very important to operacionalize the variable , cases well defined , lenguage easy to undestand and cultutral apropiated , It is necessary to validate the questionnaire in a small pilot .

I think Elias has provided a very good schema of your questionnaire: sections, and their contents.
-
So, where you reached in your study, brother?
-
your questionnaire should be based upon your aims and objectives. Also it is better to have pilot study so that you can get idea regarding its feasibility based upon time frame of your study.
-
Your questionaire should be base on the title and research that your making. Like for example are you looking the treatment of TB with diabetes or are you looking for incidence of Tb on diabetes (vice versa). There are lots of questions that can be formulated. I suggest you post your title or the main thought of your research study so we could help you out in formulating questionaires.

My question is the pronostic of women in postabortum with mifigyne

I think Elias has covered the majority of the factors you may need to cover in your questionnaire you may consider the surrounding environmental conditions in this case housing/ accomodation and type of diet one has during the treatment process.
As Mike has suggested having your concept and objectives may help in covering the critical aspects of your study
