Venous symptoms and quality of life (QOL) of 78 patients (54 women, mean age 49,5±13,3 years) with primary superficial venous insufficiency (PSVI) were compared at one year after treatment withVenous symptoms and quality of life (QOL) of 78 patients (54 women, mean age 49,5±13,3 years) with primary superficial venous insufficiency (PSVI) were compared at one year after treatment with crossectomy and stripping (C/S, 56 patients) or endovenous laser ablation (EVLA, 22 patients) using the VEINES-QOL questionnaire. Both treatments significantly (p<0,001) improved the scores for venous symptoms (difference 10,6±9,9 and 9,9±8,2 score points for C/S and EVLA, respectively) and QOL (difference 10,3±8,7 and 8,4±6,6 score points for C/S and EVLA, respectively). No difference was found between treatments regarding symptoms or QOL improvement (p=0,30). We conclude that C/S and EVLA are equally effective in improving symptoms and QOL in PSVI.

Empirical antimicrobial treatment was promptly initiated. Nevertheless a comatose state complicated the clinical course. The diagnosis of neuroborreliosis was made serologically and by molecular biology. Under adequate therapy with intravenous ceftriaxone the patient showed a slow but full recovery. This case illustrates that potentially reversible diseases need very careful decision making regarding therapeutic activities and that neuroborreliosis is a potentially reversible cause of severe neurologic impairment.

Patients with Parkinson's disease or parkinsonism are hospitalized more frequently, suffer more complications, have longer inpatient stays and are less frequently discharged directly to home. Their medication is often discontinued during hospitalization, leading to numerous complications such as akinetic crises with more falls, pressure ulcers, pulmonary problems, need for more nursing care, etc. There are various methods to overcome swallowing difficulties, and thus sustain the necessary level of dopaminergic stimulation. Commonly used anti-dopaminergic drugs to treat nausea or confusion have to be avoided.

Her laboratory results revealed a normal anion-gap metabolic acidosis, a marked hypocitraturia (0,6 mmol/24h; norm 1,6-4,5) and a urinary pH of 7,0 confirming renal tubular acidosis (RTA). We identified topiramate, our patient's medication for migraine, as the cause of the RTA. Topiramate, a carboanhydrase inhibitor leads to RTA of a mixed (proximal and distal) type and thus significantly increases the risk for kidney stones.

Gallbladder polyps are a common incidental finding in ultrasound. As non-neoplastic polyps and neoplastic polyps cannot be morphologically discriminated in ultrasound, maximal diameter of a polyp has proven to be the best predictor for malignancy. Cholecystectomy is widely accepted as the treatment of choice for patients with polyps >1 cm. Patients with polyps <1 cm should have a follow-up check for a longer period of time (with the exception of very small polyps).

Osteomyelitis caused by S. aureus can be reactivated after decades. Our patient presented with an acute episode of lower left-side abdominal pain and subfebrile temperature due to a reactivation of a S. aureus osteomyelitis in the iliac bone which was initially misinterpreted as a sigma diverticulitis. Diagnosis was established by conventional radiographs as well as CT and MR scans. Harvesting the responsible germ by a bone biopsy prior to therapy is mandatory. Therapy consists of a surgical debridement combined with a long-lasting antibiotic therapy. Relapse can be found after long latency.

Urinary tract infections in women are common. Drug resistance among Escherichia coli, the most frequent uropathogen, has increased worldwide. In a prevalence study we investigated the local antibiotic susceptibility of this microorganism in urinary specimens of three laboratories in Zurich. Resistance rates against trimethoprim-sulfamethoxazole 2010 were 28%, 16% against quinolones and 16% against amoxicillin/clavulanic acid. Resistance prevalence for nitrofurantoin and fosfomycin were low with 3,6%, resp. 0,7%. The rate of extended-spectrum beta-lactamase-producing E. coli has rapidly increased to 4,3% in 2010. Based on this data and according to the international guidelines for the treatment of uncomplicated cystitis, therapy with trimethoprim-sulfamethoxazole and quinolones are no longer recommended. Nitrofurantoin and Fosfomycin are an appropriate choice. Microbiological testing is advised.

When conservative therapies fail, minimally invasive surgical methods can be offered. The tension-free vaginal tape TVT is gold standard in the treatment of female stress urinary incontinence. In case of immobile urethra or in multi-morbid patients, the minimally invasive technique of periurethral injection of bulking agents may be useful. In patients with refractory overactive bladder, the intravesical injection of botulinum neurotoxin is available.

The disease arrived during longterm coumarin therapy because of a deep vein thrombosis and an antiphospholipid syndrome. After consideration of the differential diagnoses and due to the typical clinical picture we made the diagnosis of coumarin necrosis. We review the clinical and therapeutic features for this rare complication.
