• [Gold salts in the treatment of bronchial asthma]

  Gold salts are used mainly in the therapy of rheumatoid diseases. Thanks to their immunoregulative and antiinflammatory properties they may be used in treatment of chronic bronchial asthma. In order to study the efficacy of gold salts in such therapy the authors studied the effect of Solganal B (Schering) in 8 patients with chronic steroid dependent bronchial asthma. In 6 patients a clinical improvement was observed enabling to lower the steroid dose. Basing on this pilot study we believe that gold salts have an beneficial effect on the course of chronic bronchial asthma.

  22 normal volunteers, 57 patients with obstructive changes and 34 with restrictive type changes were studied. The following parameters were analyzed: maximal inspiratory and expiratory pressures,22 normal volunteers, 57 patients with obstructive changes and 34 with restrictive type changes were studied. The following parameters were analyzed: maximal inspiratory and expiratory pressures, occlusion pressure and components of the breathing pattern. The patients were divided according to the degree of airway resistance and fall of vital capacity. All patients compared with the healthy volunteers demonstrated higher minute ventilation, and a higher neuro-muscular respiratory drive as shown by the VT/Ti index and occlusion pressure. The breathing pattern did not correlate significantly with the different abnormalities of respiratory function in the studied groups. In the patients with obstructive changes a weak correlation was found between the occlusion pressure and airway resistance. In patients with restrictive type changes a weak correlation between occlusion pressure, static and specific compliance was found. The occlusion pressure was a more specific parameter characterizing respiratory drive compared with the VT/Ti index. Disturbances of pulmonary mechanics have a small but significant effect on respiratory control.

• [Pulmonary abscesses in material from tuberculosis and lung disease clinics of the Academy of Medicine in Gdansk]

  47 patients with pulmonary abscesses were analysed. The type of bacteria cultured from the sputum, type of chemotherapy and clinical outcome were assessed. Most of the patients (34) were already47 patients with pulmonary abscesses were analysed. The type of bacteria cultured from the sputum, type of chemotherapy and clinical outcome were assessed. Most of the patients (34) were already treated prior to admission to the Department. Most often with doxycycline and gentamycin. The following organisms were cultured: Streptococcus viridans, Escherichia coli, Staphylococcus aureus and Klebsiella pneumoniae. Antibiotic therapy was always instituted according to the antibiogram. Most often amikacin and cefamandole was given. The mean duration of hospitalization was 48 days. 44% of the patients were fully cured, in 40% only an improvement was seen. 7 patients expired (16%)--all of these patients were addicted to alcohol or had malignancy.

  A retrospective analysis of 14,461 patients examined at the Bronchological Unit in the years 1978-1988 was carried out in order to assess the incidence of bronchial tuberculosis confirmedA retrospective analysis of 14,461 patients examined at the Bronchological Unit in the years 1978-1988 was carried out in order to assess the incidence of bronchial tuberculosis confirmed histopathologically. 46 cases met the selection criteria (19 females, age range 21-75 years, mean 56.2 +/- 18.8. years, 27 males, age range 21-85 yrs, mean 50.0 +/- 17.4 yrs). Only in 22 patients acid fast bacilli were cultured. Analysis of endoscopical changes was carried out according to the Jaroszewicz-Krakówka classification. Most often diffuse inflammatory changes were seen.

  The authors present a case report of a 66 year old male that was admitted due to pulmonary lesions with cavitation localized in the upper left lobe. In the cavity an abnormal mass could be seen. In the differential diagnosis malignancy and tuberculosis were excluded. Fungi were cultured from the bronchial washings and sputum. Their morphological characteristics allowed them to be typed as Aspergillus niger. Slides from the thick, mucoid, black sputum demonstrated typical forms of the fungus. Polarized microscopy demonstrated calcium oxalate crystals. The final diagnosis was pulmonary aspergilloma caused by A.niger complicating a pulmonary abscess in the course of a pneumonia.

• [Analysis of respiratory parameters before and after working in manual laborers at a tobacco factory]

  From the group of 102 workers of local cigarette factory 51 persons had a contact with tobacco leaves before fermentation and other 51 workers with after process of fermentation. Values of spirometric parameters before and after 8 hour lung occupational exposition to tobacco leaves were evaluated in both groups. Increase of obturation of both large and small airways was found after workshift. 31 of examined persons had more than 20% inferior values of FEV1, Raw and FEF 25/75. That hyperspasmatic reaction of bronchi was found both in subjects with signs of obturation and in persons with normal results in examination before workshift. This finding concerned mostly workers who inhaled dust of tobacco leaves prior to fermentation.

  Two cases of mycoplasma pneumonias are presented. Initially this etiology was not suspected. Prior to hospitalization they were treated with cefamandole and carbenicillin and cephradine withTwo cases of mycoplasma pneumonias are presented. Initially this etiology was not suspected. Prior to hospitalization they were treated with cefamandole and carbenicillin and cephradine with gentamycin without any improvement in the clinical state of the patients. After serological tests finding a high titer of serum antibodies (1: 2560) the therapy was altered in the first patient to minocycline (in a dose of 200 mg per day for 16 days) and in the second to doxycycline (in a dose of 200 mg per day for 10 days). The first patient improved clinically after two days of altered therapy, the second after 10 days. Both patients were discharged fully recovered after 32 and 36 days of hospitalization.

  Values of VC for "theoretical subjects" of comparable sexes, age, height were found in tables and normograms according to Baldwin, Berglund, Kory, Nikodemowicz and Morris. For each series of values, mean, standard deviation, minimal, maximal, and median values were calculated. A differential test was performed to assess significance. Statistically significant differences were found. The highest values were expressed by Morris lowest by Berglund. It seems that one should rather use real values instead of predicted and new reference values are needed due to a change of height-age relation during the past 20 years.

  An analysis of the clinical course of pulmonary tuberculosis diagnosed in medical personal of the Western Pomeranian macroregion in the years 1978-1987 was carried out. Most often nurses develop this disease followed by technicians and doctors. 43% of this population did not have constant contact with active tuberculosis. Symptoms led to a diagnosis in 40% of the cases. In 27 patients radiological changes involved less than two pulmonary fields. In 17 cases a three drug regimen was instituted. The average hospitalization period was lower in these cases than in other treated in this region. Also the incidence rates of at risk medical personal were lower than overal incidence rates.

  In 49 patients with pulmonary sarcoidosis BAL differential and T-cell count, determination of IgG, IgA levels in BAL-fluid were performed.   Pleural biopsy with Abram's needle was made in 160 consecutive patients with undiagnosed pleural effusions. The sampled fluid and pleural bioptates were examined histologically, cytologically andPleural biopsy with Abram's needle was made in 160 consecutive patients with undiagnosed pleural effusions. The sampled fluid and pleural bioptates were examined histologically, cytologically and bacteriologically. In 40 cases malignant changes were found, out of 87 with confirmed malignancy. Neoplastic cells were found in 26 patients, ovreall the diagnosis was made in 65% of lung cancer cases. Out of 41 cases of tuberculosis, pleural biopsy provided this diagnosis in 24 patients (59%). Bacteriological examination and pathological of the biopsy specimen allowed to increase the diagnostic field to 63%. Pleural biopsy did not yield any diagnosis in 9 cases of circulatory insufficiency, nor in ten patients with the following diagnoses -- pleuropneumonia, reumathoid arthritis, pulmonary thromboembolism and thoracic trauma. In 13 cases the diagnosis was not made. The diagnostic yield of pleural biopsy was similar to that reported by others. It is most valuable in cases suspected of malignancy and tuberculosis.

• [Esophagotracheal fistula as a complication of radiotherapy of lung cancer]

  A case report is presented of a patient with lung cancer treated with chemotherapy followed by radiotherapy due to increasing dysphagia. A few months after radioterapy an esophago-tracheal fistulaA case report is presented of a patient with lung cancer treated with chemotherapy followed by radiotherapy due to increasing dysphagia. A few months after radioterapy an esophago-tracheal fistula developed, which was diagnosed by a radiological contrast study and esophagofiberoscopy. Post mortem examination did not reveal malignant invasion of the fistula and surrounding tissues. It seems that the fistula was a late complication of radiotherapy.

• [Long-term oxygen therapy of chronic pulmonary heart disease]

  Pulmonary artery pressure was measured in 70 patients with chronic bronchitis. Those with pulmonary hypertension underwent two hours oxygen therapy (flow 10-12 l/min). Those in whom a decrease ofPulmonary artery pressure was measured in 70 patients with chronic bronchitis. Those with pulmonary hypertension underwent two hours oxygen therapy (flow 10-12 l/min). Those in whom a decrease of pulmonary artery pressure was observed were qualified for long term oxygen therapy (LTOT). In 32 patients which were qualified for LTOT, after a two week period of 18 hour per day oxygen therapy a decrease of systolic pulmonary arterial pressure from 336.1 mm Hg to 30.9 mm Hg and a decrease of mean pulmonary artery pressure from 25.9 to 23.2 mm Hg was observed.

The above mentioned agents blocked affective aggression in rats after administration of INH. The results of this study imply that both catecholamines participate in INH induced affective aggression.
