ABSTRACT: CLINICAL CASE: A 69-year-old woman developed choroidal metastasis from a breast carcinoma 2 years after the initial diagnosis, surgery and chemotherapy. After treatment with palliative chemotherapy and external radiotherapy, we used fluorescein angiography (FA) and optical coherence tomography (OCT) to evaluate the resolution of the serous retinal detachment, as well as a later relapse in the optic nerve. DISCUSSION: OCT is useful in the follow-up of choroidal metastasis after treatment. However, OCT imaging is limited by the initial choroidal location of metastasis. Article: [Primary bilateral lipid keratopathy].

ABSTRACT: CASE REPORT: Observational, case report study. A 45-year-old woman complained of bilateral irritation and photophobia since childhood. Slit-lamp examination revealed yellow-whitish deposits involving the deep stroma of the peripheral half of the cornea in both eyes. The evolution was slowly progressive with a moderate impairment of her visual acuity. DISCUSSION: Lipid deposits adopt symmetric and bilateral configurations; the absence of corneal vascularization and ocular pathology as well as any lipoprotein disorder support the diagnosis of primary lipid keratopathy. Article: [Alterations in nocturnal melatonin secretion in patients with optic neuropathies].

ABSTRACT: To study nocturnal melatonin suppression induced by exposure to light in patients with bilateral optic neuropathies. Observational, prospective case control study. Twenty patients were included in this study and distributed in 3 groups: Group A (n=5, Healthy Control Subjects), Group B (n=10, Experimental Patients) and Group C (n=5, Blind Control Subjects). LogMAR best-corrected visual acuity, standard automated perimetry mean deviation, retinal nerve fiber layer thickness by Optical Coherence Tomography and multifocal electroretinograpy (mfERG) were used to evaluate the changes. Melatonin was analysed in the saliva by radioimmunoassay after exposure to light (600 lux for 1 hour) (nocturnal melatonin suppression test). Statistically significant differences between the groups were found. No changes in the mfERG results were detected. The nocturnal melatonin suppression test was positive in all cases in Group A, 50% in Group B and none in Group C. Half of the patients with optic neuropathies and severe visual loss were shown to suffer significant melatonin regulation anomalies, probably due to the dysfunction of the intrinsically photosensitive retinal ganglion cells (ipRGC).Archivos de la Sociedad Espanola de Oftalmologia 06/2009; 84(5):251-7. -

Article: [Surgical solution to vitreous floaters visual problem].

ABSTRACT: To evaluate the role of 25 gauge pars-plana vitrectomy (25G-PPV), after a careful patient selection, when we find highly annoying vitreous floaters and to question if this is an ethical therapeutic option. A retrospective study of eight eyes (seven patients) aged 58+/-14 years old (range 42-78) high myopes and pseudophakes who underwent 25G-PPV. Clinical data and visual acuity were studied at six to twelve months follow-up. Health-related functioning and quality of life was measured with the 39-item National Eye Institute Visual Functioning Questionnaire (NEI VFQ-39). No complications were observed. All patients were satisfied. Safety at third month was 100% and 37.5% improved one or more lines of visual acuity. Vitreous floaters can be often undervaluated by ophthalmologists, resulting in no intervention. Conventional 20 gauge PPV after a carefully examination can be an effective option for some authors. 25G-PPV incorporates also advantages as the early recovery, with little complications in pseudophakic eyes.Archivos de la Sociedad Espanola de Oftalmologia 06/2009; 84(5):259-62. -
Article: [Distribution of met-enkephalin expression in the visual pathway. Experimental study by inmunocytochemistry].

ABSTRACT: The localization and distribution of neuropeptide expression in the cat visual pathway can provide information about the function of that pathway. Study of optic pathway in eight cats. Following extraction of the brain, slices were prepared using a microkeratome. The slices were examined by indirect immunocytochemistry using anti-metenkephalin as antibody to determine the presence or absence of this pentapeptide in the visual pathway. Met-enkephalin receptors in both cortical and subcortical regions of the brain were detected. This suggests that met-enkephalin could be involved in the visual mechanism. Article: [Post-traumatic striate melanokeratosis in a Caucasian woman].

ABSTRACT: CLINICAL CASE: A fair-skinned woman presented marked striate melanokeratosis in her left eye related to recurrent corneal erosion. The source of pigmentation was a conjunctival melanosis. The conjunctival melanosis responded to treatment with topical mitomycin, while the corneal pigmentation persisted. DISCUSSION: Striate melanokeratosis is a condition described in dark-skinned patients who show a well-defined pigmentation of the limbal area, with only one case of striate melanokeratosis reported previously in a Caucasian person. The stimuli for this proliferation are corneal lesions or melanosis close to the limbus. Article: [OCT in acute anterior uveitis].

ABSTRACT: To evaluate macular thickening, the state of the optic disc and retinal nerve fibers layer (RFNL) in patients with an actual episode of acute anterior uvetis (AAU) by optical coherence tomography (OCT), and compare them with a control group. In this prospective, cross-sectional, observational and controlled study we recruited 27 consecutive eyes of 20 patients with an actual episode of AAU, age and sex-matched with 40 healthy eyes of 20 volunteers. The age ranged between 8 and 78 years old, and all were evaluated by the , and scans by OCT. The patients were evaluated twice in a period of 1 month in order to obtain an average between the two measurements by OCT. We found a statistically significant increase in macular volume in AAU eyes compared with control eyes. Total macular volume in uveitic eyes was 7.3 SD 0.6 mm(3) and in healthy eyes was 7.01 SD 0.3 mm(3) (mean SD standard deviation) (p<0.001). Indeed, we found a statistically significant increase in the superior RFNL thickness compared with controls. The Smax/Imax measurement was 1.05 SD 0.1 in pathological eyes and in healthy eyes it was 0.97 SD 0.1 (p<0.02). We found by OCT that patients suffering an AAU showed an increase in macular volume and superior RFNL thickness versus control eyes in the acute episode.Archivos de la Sociedad Espanola de Oftalmologia 05/2009; 84(4):185-90. -
Article: [Retinopathy of prematurity in multiple births: risk analysis for plus disease].

ABSTRACT: To analyze the risk factors associated with plus disease in retinopathy of prematurity (ROP). Over a period of 8.5 years we carried out a prospective study of ROP in twins and triplets. Fifty-four multiple-birth infants with low birth weight (< or =1500 g) and low gestational age (32< or = weeks) were admitted to the University Hospital of Granada. Logistic regression analyses showed the following variables to be associated with an increased risk of plus disease: severe ROP, large area of avascular retina, low gestational age, low birth weight, a patent ductus arteriosus, length of mechanical ventilation, adverse events increase, low 5 min Apgar scores and poor postnatal weight gain (in the first 4 to 6 weeks of life). Using multiple logistic regression, only the grade of ROP (OR: 5.5; p < 0.009) and poor postnatal weight gain (OR: 0.58; p < 0.04) were predictive factors of development of plus disease. Infants with disease gained an average 3.9 +/- 3.1 g/day in the first 6 weeks of life, compared to a mean of 11.84 +/- 8.3 g/day for those without plus disease (p < 0.0001). Advanced ROP stages and poor weight gain were the most significant factors associated with plus disease. Twins who gained weight at more than 7 g/day in the first 4-6 weeks of life had a significantly reduced risk of plus disease. Article: [Use of non-mydriatic retinography to determine the prevalence of diabetic retinopathy in diabetic patients].

ABSTRACT: To establish the prevalence and severity of diabetic retinopathy (DR) in a sample of patients from "Health Area 3" in the Community of Madrid using non-mydriatic retinography and telemedicine. All patients who had their ocular fundus examined due to one of the following conditions were included in the sample: type 1 diabetes (DM1), type 2 non- insulin-dependent diabetes (DM2NID), and type 2 insulin-dependent diabetes (DM2ID). In all cases, 3-field retinographies were taken of both eyes to be evaluated for the presence of DR, and its severity, by an ophthalmologist at the hospital. No DR was observed in 79% of the patients examined, 9% had a mild non-proliferative DR, 9% had a moderate non-proliferative DR, and 1% had a severe non-proliferative DR. The estimated prevalence of DR in "Health Area 3" of the Community of Madrid was thus 21%. In 12% of the examinations the retinographies were considered "low quality", thus requiring a second evaluation at hospital. Article: [Descemet membrane endothelial keratoplasty (DMEK): two-year results].

ABSTRACT: To report the two-year results of Descemet membrane endothelial keratoplasty (DMEK) for managing corneal endothelial disorders. Non-randomized prospective clinical trial. A DMEK was performed in ten patients with Fuchs' endothelial dystrophy or bullous keratopathy. A 3.5 mm clear corneal incision was made and "under air" DM was stripped off from the posterior stroma. A 9.0 mm diameter, organ cultured donor DM roll was inserted into a recipient anterior chamber, positioned into the posterior stroma and secured by completely filling the anterior chamber with air for 30 minutes. Three eyes showed complete detachment of the tissue; this was managed by a secondary Descemet stripping endothelial keratoplasty procedure. The remaining seven eyes had a best corrected visual acuity of >or=0.7 in three eyes (43%) at one month, in five eyes (71%) at six months, and in six eyes (86%) at one and two years. At six months, the endothelial cell density averaged 2039 (+/-373) cells/mm2 (n=7), at one year 1925 (+/-267) cells/mm2 (n=7) and at two years 1730 (+/-400) cells/mm2 (n=6). DMEK may provide quick and nearly complete visual rehabilitation. Since the donor tissue can be stripped from donor corneo-scleral rims, the procedure may be readily accessible to most corneal surgeons.Archivos de la Sociedad Espanola de Oftalmologia 05/2009; 84(5):237-43. -

Article: [Pterygium surgery: comparative study of conjunctival autograft with suture versus fibrin adhesive].

ABSTRACT: To compare recurrence rate, complications, and biomicroscopical findings after conjunctival autograft pterygium surgery with the use of a fibrin glue (Tissucol Duo, Baxter AG, Vienna, Austria) or suture (7-0 silk). Patients with nasal pterygium were included in two groups of conjunctival autograft surgery. In 9 of them the graft was sutured to the surrounding conjunctiva and in 8 of them the graft was fixed to the conjunctiva using fibrin glue. 17 patients (17 eyes) 41.2% women and 58.8% men were surgically treated. Mean patient age was 59.8 years. Five of the patients presented recurrence for simple excision and 12 presented primary pterygium. The extent of corneal invasion was 2 to 4 mm. In the suture group, 33.3% of the patients experienced pain after surgery compared to none in the fibrin glue group. In the suture group, 44.4% of the patients presented inflammation after surgery compared to none in the glue group. One patient from the glue group lost the graft and presented a recurrence one month later. The use of fibrin glue in pterygium surgery reduces patient symptoms, inflammation and discomfort. ABSTRACT: CASE REPORT: We report the case of a 48-year-old man with diffuse diabetic macular edema of recent progression treated with two consecutive intravitreal injections of bevacizumab followed by focal laser therapy, with good anatomic and functional response. DISCUSSION: Diffuse diabetic macular edema is difficult to manage and frequently requires multiple therapies. Article: [Development of conjunctival hyperemia with the use of a fixed combination of latanoprost/timolol: systematic review and meta-analysis of clinical trials].

ABSTRACT: To asses the association of conjunctival hyperemia with the use of a fixed combination of latanoprost/timolol, through a systematic review and meta-analysis of clinical trials in patients with glaucoma. A systematic review of published clinical trials of latanoprost/timolol and other competitors was conducted in Medline, Embasse and Cochrane Controlled Clinical Trials Register, between 2000 and 2007. Statistical analysis included calculation of the odds ratio (OR) with its 95% confidence interval (CI) using the fixed effects model of Mantel-Haenszel and the random effects model of Der Simonian and Laird. To assess the heterogeneity between trials the Cochrane Q test and the I(2) rate were calculated. The conjunctival hyperemia rates obtained were compared with the Chi-square test. A total of 8 clinical trials comparing latanoprost/timolol fixed combination with different therapeutic options were found. As trial heterogeneity was moderate (Q: 14.64; df=7; p=0.041; I(2)= 52.2%) a random effects model was used. The final OR was 0.47 (CI 95%: 0.24-0.90); p = 0.024. The total conjunctival hyperemia incidence was 2.9% in the latanoprost/timolol group and 7.0% for the competitors (p<0.0001). The use of a fixed combination of latanoprost/timolol is associated with a significant reduction (53%; CI 95%: 10%-76%) in the development of conjunctival hyperemia against the other compared options for the treatment of glaucoma.Archivos de la Sociedad Espanola de Oftalmologia 05/2009; 84(4):199-207.
